printedsolid - I'm probably with Vinnie.
I do a ton of experimentation with different settings and materials. Often times beta materials from the manufacturer. In these situations, it's probably 40% or higher.
Beyond that, I don't have many failures.

It's worth mentioning that I regularly check belt tension, bed level, and tend to proactively clean nozzles at least every few weeks or more often if I'm running filled materials.

ashley@3dprintvancouver.com - Very rare and usually my fault, or at least I knew I was risking and went ahead anyway. Happened the other day. I was nearing the end of a spool where the filament was wound pretty tight and wondered if I should risk it and run another print. I tossed the dice and lost.

raykholo - Used to be very high due to an assembly error in the extruder arm, the bearing didn't exert enough pressure on the filament into the drive gear. Long since fixed and issues are now practically non existent. As long as your first layer is put down well, that's all that matters.

So I'll go with that less than <1% figure, clarifying that it took me months of tinkering to get my machine to the point that it is at today and that sometimes filament from a new supplier can be finicky until the correct settings are tuned in.

defcon3d - We produce 225+ of the same pieces every week. We average about 1% failure rate.
The key is to be proactive with your maintenance plan not reactive.

nyl0cke - Ever since I got my PEI build plate, all small prints haven't had an excuse to fail. Having some troubles figuring out the exact values for my tower offsets (delta geometry yo) so on the edges of my bed things rise and fall up to .1mm. Only reason I have issues, and an influx of orders have kept me from figuring it out. I'm sure glad there is a raft feature in Cura..

elpulpo - I'd say somewhere between 5 and 10 but then again, I always watch the first couple of layers and abort at the first sign of trouble, so I wouldn't count any aborted prints, which would probably take it up towards 20%.
