This is a 40 mm fan mount for the electronics on a Mendel90. But it should be usable wherever you need to screw a rotatable fan to wood.Use an m3 screw to attach both parts together. The holes are 20 mm wide, so make the screw 25 mm or so. You will also need 3 m4 screws to attach the pole part to the frame.
